Transconjunctival blepharoplasty, also known as fat redistribution under the eyes or hernia removal, is a well-known surgical procedure that helps patients get rid of dark circles and bags under their eyes. In Russia, this procedure is typically performed after the age of 35, while in South Korea, it is offered to patients of all ages.
One of the major advantages of transconjunctival blepharoplasty is the absence of visible stitches and the need for their healing. The incisions for fat redistribution are made on the inner side of the eyelids using a laser and are not visible on the face. In rare cases, the surgeon may also make 1-2 tiny stitches, about 1mm in size, on the outer side of the skin in the cheek area, but there is no need to worry about them—they appear as small light acne scars after just 2 weeks and completely disappear within 2-3 months.
Frequently, the procedure is combined with fat grafting because the redistributed tissues may be insufficient to make the skin under the eyes smooth and eliminate the bluish tint.
For patients over 30 years old, a combination of fat redistribution and lower blepharoplasty may be recommended, as there may already be excess skin tissue and wrinkles under the eyes. In this case, the incision will be more noticeable and will pass slightly below the lash line.

Why do patients consider transconjunctival blepharoplasty?
-
Prominent eye swelling: In some cases, this may be related to allergies or more serious internal organ conditions, but often it is a genetic feature of appearance that makes the eyes appear inflamed and tired. In such cases, surgeons recommend the procedure immediately after reaching adulthood, as a few years later, the skin may stretch, wrinkles may appear, and the procedure may need to be combined with lower blepharoplasty and Botox injections.
-
Dark circles around the eyes: Sometimes, dark circles may be due to a lack of fat under the eyes. In this case, fat grafting is recommended. Additionally, some patients naturally have darker skin under their eyes. In such cases, skin lightening procedures in cosmetic dermatology can help. However, if the circles are caused by uneven distribution of fat tissues under the eyes, transconjunctival blepharoplasty can easily correct this issue.
-
Under-eye bags: Bags under the eyes can result from improperly administered fillers or the accumulation of fat tissues. If fillers have been used, it is best to consult a dermatologist for filler-dissolving injections without delay, as the skin above the filler can stretch and look swollen even after filler removal. In all other cases, fat redistribution under the eyes can solve the problem.
Who is a suitable candidate for the procedure?
Transconjunctival blepharoplasty is suitable for individuals who want to eliminate under-eye bags without noticeable external scars. This procedure is ideal for those who do not have significant excess skin on the lower eyelids but have fat deposits causing puffiness or swelling. The best candidates are healthy adults with good skin elasticity who wish to improve the appearance of their eyes while maintaining a natural look. This procedure is particularly suitable for those seeking a minimal recovery period and highly natural results.

Preparing for blepharoplasty in South Korea
Before undergoing blepharoplasty in South Korea, it’s important to prepare adequately:
-
Fasting: Refrain from eating and drinking for 8 hours before the procedure and take a shower.
-
Medication: Avoid medications like Aspirin, Ibuprofen (including Advil or Motrin), and others that can increase the risk of bleeding for at least 2 weeks before surgery. Tylenol should be avoided for 3 days before the operation.
-
No smoking: Smoking, including hookah and e-cigarettes, reduces blood flow to the skin and can slow down tissue healing. Many surgeons recommend quitting smoking before the surgery due to the high risk of complications.
-
Medical history: Review your medical history and be prepared to answer questions about current and past illnesses and any recent medications you’ve taken. Don’t hide this information from your surgeon and coordinators.
-
Consultation: Visit the surgeon for an examination and show them photos of your desired results, preferably without makeup, to ensure a natural look.
-
Medical tests: If you haven’t had recent medical tests, they will be conducted in Korea.
-
Transportation: Arrange for someone to drive you to the clinic and back after the surgery.
-
Essentials: Prepare items you may need during your recovery, such as tissues, lip balm, water, a chocolate bar, or your mobile phone, and keep them by your bedside.
The blepharoplasty procedure
During the procedure, after detailed planning and patient agreement, sedative anesthesia is administered to the patient. The patient falls asleep, and the surgery begins. An assistant lifts the lower eyelid away from the eye, creating a small opening for the surgeon to access the inner eyelid.
Using a laser and medical sutures, the surgeon removes pieces of the patient’s fat and repositions them to create a smooth surface without protrusions or depressions. After completing the surgery, absorbable stitches are applied, and medical tapes are attached to the skin. The patient is then transferred to a recovery room for rest and rehabilitation.
In conclusion: South Korea stands out as a top destination for cosmetic surgery, especially blepharoplasty, due to its highly skilled surgeons, advanced medical technology, and accredited clinics. Proper preparation before the surgery is essential to ensure a successful outcome. If you’re considering blepharoplasty in South Korea, follow these guidelines and consult with a qualified surgeon for the best results.
What to expect after surgery
After the surgery is completed, the translator and medical nurses will wake you up and assist you in moving to your room. Most patients do not remember this moment, as they often feel like they woke up in a warm bed, thanks to the effects of anesthesia.
In the first 10-20 minutes after waking up, you may experience chills, but this will quickly pass. A warm mattress pad will be placed on your bed to make this period more comfortable. You will experience minimal pain, as it is dulled by pain-relieving injections and medications used during the surgery. However, you may feel some swelling in the upper eyelids as your body begins the regeneration process. Medical patches will be placed on your face; do not touch or remove them to see the results, as this is strictly prohibited.
A few minutes after waking up, you will be given a cold compress to place on your eyes. The more frequently you use the compress during the first week after the surgery, the faster the swelling will subside.
You will remain in the clinic for 1-2 hours after the surgery. You will have a catheter in your arm to administer medication. After the IV is removed, the coordinator and medical nurses will check your condition. If you do not experience symptoms such as dizziness and nausea, you can prepare for discharge by changing clothes, collecting your medications, and receiving post-operative instructions.
Before bedtime on the evening of the surgery, place several pillows under your head to reduce swelling and take the medications prescribed by the surgeon, as they contain pain relievers. Apply a cold compress every few minutes. Tears may be released from your eyes; gently wipe them with a tissue.
Over the next three days, the swelling may increase, and your upper eyelids may become puffy. Bruising and slight bleeding may occur. Do not be alarmed and continue to apply cold compresses and follow the rehabilitation guidelines. After an examination by the surgeon on the 7th day after the surgery, the swelling will begin to subside, and the medical patches will be removed. You will be discharged without bandages and patches.
